These resources are intended to help teams understand ClinFIT, prepare for data collection, train local staff, and contribute to the global ClinFIT initiative in a standardized and consistent way.
ClinFIT Excel Template for Data Collection
To support participating centres, we provide a simple Excel template that clinicians can use to collect ClinFIT data in a standardized way. The template is designed to make data entry practical, consistent, and easy to integrate into routine rehabilitation workflows.
Clinicians can use the template to record ClinFIT item ratings at the relevant time points, such as admission, discharge, or follow-up. Each item is rated using the standard ClinFIT response scale:
ClinFIT rating scale
0 = No impairment
1 = Mild impairment
2 = Moderate impairment
3 = Severe impairment
4 = Complete impairment
8 = Not specified
9 = Not applicable
This template helps ensure that data are collected in a comparable format across clinics and countries, supporting the global ClinFIT initiative and contributing to stronger evidence on functioning outcomes in rehabilitation.
